SmileDirectClub shutdown raises questions—here are some answers

From Time Fortune:

SmileDirectClub, the company that once promised to revolutionize oral care with clear dental aligners, has filed for bankruptcy and announced it’s closing its global operations. Existing customers are left without support and answers, and may not receive refunds. Multiple dental organizations warn about safety concerns with “direct-to-consumer” dentistry. Seek local orthodontic care.

SmileDirectClub lost $86.4 million in 2022 and filed for Chapter 11 bankruptcy, reporting nearly $900 million in debt. The company struggled to stay competitive due to legal battles, stock price drops, and unprofitable years. The company was unable to find a partner to keep it afloat and has ceased operations, leaving existing customers without support and with possible financial losses.

Existing SmileDirectClub customers have been notified that the company’s telehealth aligner treatment is no longer available. Orders that haven’t shipped have been canceled, and the “Lifetime Smile Guarantee” no longer exists. Those who have made payments and signed up after the bankruptcy filing may have higher priorities for refunds, but it’s uncertain whether they will receive compensation. Customers are urged to consult their local dentist or orthodontist for further treatments.

Orthodontic professionals caution about the health consequences of direct-to-consumer aligner treatments, which don’t require in-person visits to a dentist. Such treatments overlook key aspects of oral health and can lead to harm if not properly monitored. Many former direct-to-consumer aligner patients end up coming to orthodontic practices for reevaluation, presenting gum disease and other issues. The lack of direct supervision and patient-doctor relationship raises public health and interest concerns for professionals.

The dental industry has always expressed opposition to direct-to-consumer aligners, due to the risks involved with not having in-person evaluations and direct supervision. SmileDirectClub and similar providers offer convenience and affordability, but it’s crucial to consider the potential harm and consequences of such treatments. Orthodontic professionals note that teledentistry can be appropriate within safety guidelines, such as remote monitoring between treatments. However, patients should maintain a direct patient-doctor relationship and be seen in-person before starting permanent orthodontic treatment.
